How to Request Your Medical Records

• By Phone:
  Call us at (760) 873-2152. Our knowledgeable team is available to guide you through the process, answer any questions, and ensure you have all the information you need.

• By Fax:
  Fax your request to (760) 873-6734. This method offers a reliable and safe way to submit your request, with prompt processing by our team. Please call us for direction on how to best submit your request by fax.

• In-Person:
  Visit our office in the Administration Building at 150 Pioneer Lane, Bishop, CA 93514. Our friendly staff is ready to assist you face-to-face, ensuring a smooth and comfortable experience.

Office Hours

Our HIMS Department operates Monday through Friday from 8:30 a.m. to 4:30 p.m. We recommend contacting us during these hours for the most efficient service.
